nurses, and other school support staff to ensure the
safest possible environment for students. By working
together we can create a plan that can help meet your
student's special dietary needs.

Life Threatening Food Allergies:
Washington State law requires that all students with life threatening
health conditions have the needed medical orders, medications/equipment,
and a nursing care plan in place before the child may attend

Special Diets/Doctor's Signature:
If your child has a medical dietary need that requires
food to be different or to be modified, please contact us
by phone or email.
To make menu modifications or changes for your child,
we are required to have a Diet
Prescription Form filled out by your doctor.
The Diet Prescription Form is available at your school office
or by calling Nutrition Services. This form is for non-life
threatening food substitutions usually due to a food intolerance
(i.e. substitute juice for milk).

They can be checked out for two weeks. Call you school
nurse or Nutrition Services at 354-7270.
