We want to create a partnership with parents, principals, doctors, teachers, nurses, and other
school support staff to ensure the safest possible environment for students. By working
together we can create a plan that can help meet your student's special dietary needs.
Special Diets/Doctor's Signature
If your child has a medical dietary need that requires food to be different or to be modified,
please contact us by phone or email.
Have the appropriate form filled out and signed by your doctor. Below are the forms needed for each
special diet request. Please feel free to call our office at 354-7270 if you have any questions.
