Insurance Information - This form must be completed and signed by a parent indicating the athlete is covered by health insurance. If you do not have insurance, school insurance must be purchased. Student Health Conditions and After School Activities It is the parent or guardian's responsibility to alert all after school activity coaches or instructors if their child has a health condition and/or a Care Plan in place. This includes, but is not limited to: sports, drama, debate, music, cheerleading, and dance. It is also the parent or guardian's responsibility to provide any medication needed for these after school activities. Some of these programs are led by individuals who are not full time employees of Spokane Public Schools. They may not have access to the health trainings offered through the district. Please make sure that your child's coach or instructor knows how to respond to your child's health condition in an emergency. |
The athletic department of Ferris High School holds with the following beliefs regarding the use of performance enhancing supplements (e.g. creatine, protein, testosterone) by our student athletes.
ü We believe schools and coaches are not medical authorities and should not recommend or promote any kind of non-prescription medication or nutritional supplement.
ü We believe it is in the best interest of each student athlete to seek the advice of their own personal physician. We believe the physician, the athlete’s family, and the athlete must make an informed decision as to the use of performance enhancing supplements.
ü We believe that in order to become a complete student athlete, the individual must understand and exercise a strong work ethic.
ü We believe that individual effort is the most important ingredient to athletic competition, not winning or losing. It is our hope that through this work ethic, the athlete will develop true commitment, dedication, and passion for their sport. This includes NOT taking shortcuts to achieve their athletic goals.
