2008 - 2009 Chase Athletics Program

Please note:

Sports physicals are required before students can physically participate in any practice. (Physicals are valid for 24 months from the date of the exam and must then be on file with the Athletic Coordinator.)

  10 practices are required before a student is allowed to participate in an athletic event.
  Students need to be passing all classes with a G.P.A. of 2.0 and NO “F"s in order to participate in an athletic event.

Does your student want to participate in a particular sport? Please select from the following list, print and sign the form, and deliver to the Chase athletic department. Be sure to print an extra copy for yourself for future referral.

(Participation forms for baseball/softball, track, and basketball will be available two weeks before the season begins)
