Before athletes are eligible to attend the first day of athletic each athlete must return a completed athletic packet to the Business Office. These forms must be returned in person. Once your athletic packet is approved, the Business Office will issue a CLEARANCE FORM for the athlete to take to the first day of practice: no clearance form means no practice.

Following is the list of forms needed for a complete athletic packet (All files are in PDF format ):

1. Physical form completed, signed and dated by physician, parent and student (valid for 24 months)
2. Insurance Waiver, signed by parent - or - purchase school insurance
3. LC Eligibility Form, signed by parent and student
4. Activity Agreement Rules and Regulations, signed by parent and student
5. Individual Sport Safety Guidelines, signed by parent and student

Please print and complete each form. Once the forms are completed, please return all 5 forms to the Business Office.
