athletes must return a completed athletic packet and obtain a
clearance slip from the NC Business Office before they are eligible
to practice--no clearance means no practice. The following forms
are available in PDF format. To receive your clearance slip, please
print, complete, and submit the following forms in person to the
Business Office:
Physical
Form completed, signed and dated by physician, parent and
student (valid for 24 months)
Insurance
Waiver, signed by parent - or - purchase school insurance
NC
Eligibility Form, signed by parent and student
Activity
Agreement Rules and Regulations, signed by parent and student
Individual
Sport Safety Guidelines (listed below), signed by parent and student
