Health Care Provider Orders & Forms

Please be aware that all students in Spokane Public Schools are required by state law to be fully vaccinated prior to the first day of attendance (RCW 28A.210.080). If the student’s immunization record is not complete, the student will be excluded from school. If your child is exempt from vaccination, please ensure that your school receives this documentation. The requirement to be fully vaccinated does not include the COVID vaccine.

This is a notice of you and your child’s rights under Section 504 and the rights you have if you disagree with the school district’s decisions.

All students who have asthma must have an Asthma Care Plan and Medication Orders AND an inhaler at school before the first day of attendance. They MUST have an inhaler or other rescue medication at school or they will be excluded.
