2021-22 Enrollment Application Steps and Important Information

• Create an account at Survey Monkey Apply (accessed by the application link that goes live February 8th)
  • Enter the student's name in the first and last name fields  - not the guardian's name
  • A verification email will be sent to the email entered in the email field
    • The verification email might be sent to your junk or spam folder

• Complete and submit your 2021-22 Enrollment Application
• Very Important! You must complete the following two tasks in order for application to be 100% and processed efficiently for placement: 
  • Task One: Complete the Application in the portal
  • Task Two: Complete the Request for Recommendation in the portal
  • After you complete the Request for Recommendation please click the submit button and then click on Mark as Complete (these will be located at the bottom of the screen)
  • Next submit your completed application by clicking on submit (left side of the screen) afterward a box will open – click submit again
    
    
• Automatic emails will be generated and sent as your application moves through the stages of review/placement
• The emails will be sent to email addresses imputed for the student, guardians, and the counselor at the sending high school.
• The email might be sent to the junk/spam folder
