Immunization records and healthcare plans
Some items that may need early attention are updating immunization records or a certificate of exemption and healthcare plans, which must be received at least 10 calendar days before the start of school. Health forms should be turned in to school offices before Friday, Aug. 22, 2025.
By state law, students who do not have current records will be excluded from school until documents have been received and processed. If you have questions, please reach out via Let’s Talk.
Sports physicals
Middle and high school students who are participating in athletics will also need a current physical exam on file to participate; sports physicals are good for two years.
CHAS Health’s School-Based Health Centers are hosting Sports Physical Days in August to provide no-charge sports physicals for Spokane Public Schools students. Advanced scheduling is recommended. Students do not need to be enrolled at the SBHC’s location to receive services.
