On January 7th, from 3:30-5:30pm, Salk will be offering free physicals at the school. Please fill out the registration form below and bring it back to school when we return from break. We would like the forms back prior to the date, but will also take same day forms. A parent or guardian needs to attend the day of the physical. Otherwise, we will need a note of authorization for the student to receive the physical. A parent or guardian will also need to be available by the phone is case there are any questions.
